What a decision tells you and how to follow it downstream.

Each decision carries an identity and an evidence hash. Carry both downstream so that the action your agent executes can be tied back to the decision that permitted it.
- The state: ALLOW, BLOCK or ESCALATE.
- The decision identity, used to look the decision up again.
- The evidence hash returned with the decision.
